Insurance

Fees, Payment and Insurance

It is important that you check with your insurance carrier for eligibility prior to your examination. We will gladly assist you with this request, although it may be required for you to contact your primary care physician for a referral. If you have an insurance card or have any changes with your insurance, please notify us upon arriving to your appointment.

We participate in many different insurance plans including Aetna, BC/BS, CHP+, Cigna, Medicaid, Medicare, Eyemed, PacifiCare, Rocky Mountain UFCW, Secure Horizons, Sloans Lake, United HC, Vision Service Plan, Beech Street, First Health, GEHA, Great-West, Humana, Sloan's Lake, Vision Benefits of America and several others. Since requirements change almost as quickly as the weather, our list of accepted and participating insurance changes periodically. If we are unable to bill your insurance, you will be provided with an itemized statement that you will be able to send to your insurance for secondary reimbursement. For your convenience we accept cash, checks, Visa, MasterCard, Discover, American Express and CareCredit. If you are unable to pay the full balance of your account, our office manager Debi will work with you for acceptable payment arraignments.


Our patients can choose to utilize the financing options provided by CareCredit. Please ask for a brochure or visit the CareCredit Website.

If you do not find your insurance carrier listed above, please feel free to contact our office for an updated listing. If we are not accepting your insurance at the time of your inquiry and would like us to begin working with your insurance, our office manager Debi will contact your insurance for the necessary requirements. If we are able to accept your insurance, it may take as long as 60 to 90 days before we receive an answer. We are always looking for better ways to serve our patients.
